Important Information

• If you already have a form that was filed since July 1 for the current fiscal year, no additional form is needed if the information is correct. Please use the "Search E-CIQ Form" feature to view your form.

• If you do not have a form on file since July 1 or if your form is no longer correct, please use the "Submit E-CIQ Form" to have a new form submitted.

• If you need to make a disclosure involving more than one (1) local government officer, then submit a separate E-CIQ form for each local government officer.
